Job Title: Care Manager
Location: New York State
Reports To: Care Manager Supervisor
Position Type: Per Diem, Field-Based (Client Homes Only)
Position Summary:
Care Managers provide client-centered, strength-based support and service coordination to adults and children enrolled in the Health Home program. They advocate for clients' well-being by identifying needs, linking them to services, and ensuring ongoing support. Responsibilities include home visits to clients, provider and resource coordination, EMR documentation, and regular communication with clients and families. Clients are located throughout New York State.
Key Responsibilities:
- Outreach, enrollment, and engagement of eligible clients
- Completion of CANS-NY assessments (passing score: 70)
- Development and maintenance of individualized care plans - Accurate EMR documentation aligned with service plan goals
- Assistance in accessing medical, behavioral, and social services - Collaboration with families, providers, and care teams
- Monitoring utilization, addressing barriers, and maintaining compliance
Required Qualifications:
- Bachelor's degree in any field - Minimum 2 years of healthcare or care coordination experience
- Must pass background checks (SEL, SCR, & CHRC if applicable)
- Completion of 1- 1½ -week training and proficiency in EMR systems Compensation:
- Per diem based on case acuity:
* Low acuity: $200/month per client
* Medium acuity: $210/month per client
* High acuity: $300/month per client
- Maximum of 60 points per Care Manager
Job Types: Full-time, Part-time
Pay: $200.00 - $300.00 per month
