About Job

Position: Social Work Case Manager

Location: Confidential location in Queens, NY

Hours: Part-Time (up to 20 hours/week, flexibility for afternoons, evenings, and weekends)

Salary: $40/hour

LifeWay Network seeks a compassionate and skilled social worker with prior experience supporting survivors of human trafficking. This part-time position is in-person in our confidential safe home located in Queens, NY.

LifeWay Network
is a 501(c)(3) nonprofit organization that joins the global movement against human trafficking by providing safe housing for women who have been trafficked and offering education to stakeholders on the issue. LifeWay Network (LWN) envisions a world in which human trafficking is eradicated and every survivor is strong, connected, and free.

Founded in 2007, LWN is a New York-based nonprofit that addresses two critical needs in the anti-trafficking movement: educational programming to raise awareness of trafficking and therapeutic safe housing for women survivors of all forms of trafficking.

LifeWay Network’s Safe Housing Program
LWN serves both domestic- and foreign-born, female-identifying survivors of human trafficking, accompanying them through healing. LifeWayNetwork’s safe housing program has welcomed more than 150 women from more than 40 countries in our safe houses in New York State.
What sets LWN’s Safe Housing Program apart is its community living model. It offers a safe home by welcoming each woman into a supportive, caring community of residents. This host community helps her recover from her trauma, regain her sense of self-worth, and enables her to move from isolation towards reclaiming her independence.

Reports to:
Senior Safe House Social Worker

Role Description
: LifeWay Network seeks a Part-Time Social Worker to provide comprehensive, trauma-informed support services to survivors in our safe housing program. This position requires a compassionate, trauma-informed, and culturally sensitive individual with strong clinical and organizational skills. The role offers up to 20 hours per week, ideally spread across 3 to 4 days, providing flexibility for afternoons, evenings, and weekends.

Responsibilities:

  • Initiate and maintain positive, professional, and mentoring relationships with survivors and staff following LifeWay Network’s Mission, Principles, and Core Values.

  • Process referrals and intakes for potential new residents, ensuring that new residents meet the eligibility criteria.

  • Welcome new residents to the home and support their adjustment to the daily routine of LWN’s community living environment.

  • Provide ongoing trauma-informed, culturally sensitive counseling and support to residents, including needs assessments, safety planning, and crisis intervention.

  • Promote a peaceful and healing environment by incorporating the principles and practices of trauma-informed care into all aspects of work, including crisis prevention, intervention, and de-escalation.

  • In collaboration with the survivor, House Manager, Safe House Staff, and support service agencies, oversee the development and implementation of individualized care plans for a healthy, safe transition to self-sufficiency, stability, and reclaiming their independence.

    • Service plans should be completed with residents within 2 weeks of their move-in date, setting goals and tracking progress throughout their residency at LWN. Service plans should be updated and revised every 3 months to ensure residents are on track to meet their personal goals.

  • Maintain accurate and timely records of resident interactions and service provision by completing case notes and adding any relevant information to shift reports, for staff to review ahead of their shifts.

  • Provide direct services, including survivor advocacy, crisis intervention, safety planning, general intake, service assessments, referrals to appropriate legal and long-term support services, and coordinating ongoing support services both onsite and with community partners.

  • Provide individualized care, support, and follow-up to help survivors access community resources and achieve care plan goals, including specialized counseling, physical and behavioral health care, educational advancement, employment, and life skills in conjunction with referrals to necessary legal and social services.

  • Provide case management to residents who are not connected to a case management agency outside of LWN, to ensure residents are connected to comprehensive support services outside of the safe house program.

  • Participate in weekly case reviews and communicate regularly with the Safe House Manager and staff to ensure coordinated delivery of service.

  • Maintain accurate and current information in both electronic and paper case files for all residents, ensuring easy access for all LWN staff via Google Suite concerning the survivor's care.

  • Oversee the completion of appropriate documentation (i.e., progress and transition notes; significant event and critical incident forms; overnight and discharge planning; meals, chores, and room inspection forms, etc.).

  • Work collaboratively with other staff members, service providers, and community partners to ensure comprehensive support for residents.

  • Work with residents to navigate systems and advocate for their rights and needs.

  • Adhere to ethical guidelines and standards of practice in social work. Maintain confidentiality and discretion, and complete all job functions in an ethically and culturally competent manner.

  • Attend all trainings and commit to continuous professional development and integration of training and tools into individualized tasks and responsibilities.

  • Provide support and training (as needed) to Safe House Staff to ensure a trauma-informed and culturally sensitive environment for residents.

  • Help coordinate workshops, field trips, and support groups for residents in coordination with the Senior Social Worker and the House Manager.

  • Other duties as requested and needed.

Qualifications:

  • Demonstrated ability to be a self-starter and work independently and within a team.

  • Excellent written and verbal communication skills and listening skills.

  • Demonstrated success with community relationship-building.

  • Heart, humor, and a commitment to the LifeWay Network mission.
