The State Crossing Support Coordinator provides on-site support to clients living in a recently-built apartment building of 30+ units. They provide case management and coordinate services on behalf of these clients with other community partners. On a daily basis, the State Crossing Coordinator interacts with clients , engaging them through creative and resourceful strategies, building trust, and maintaining documentation to reflect this engagement. At times, clients may be escalated or in crisis. The State Crossing Coordinator will intervene, respond to emergencies, and initiate appropriate action as required. This sometimes includes contact with emergency response systems, if necessary.

The State Crossing Support Coordinator is part of the Outreach Team but primarily works at the State Crossing location. While most of the work is performed on-site, there are some remote working opportunities for this position.

Core Responsibilities Include:

  • Provide ongoing, active outreach and creative engagement to tenants
  • Conduct comprehensive assessments and help tenants develop action plans to achieve goals
  • Work with tenants and property management to coordinate eviction prevention efforts and develop housing permanency plans
  • Negotiate and advocate with external community resources providing medical, mental health, substance use, and other supports
  • Assist tenants with conflict resolution among tenants and neighbors
  • Work to build community and peer support among tenants, including participation in community programming activities
  • Meet documentation requirements as dictated by program need
  • Work with clients in their homes, in community centers, and in the larger community
  • Participate in on-call rotation and occasional evening and weekend work as necessary
  • Assist tenants as needed with activities of daily living such as transportation, housekeeping, meal preparation, medication, shopping, laundry, or other hands-on assistance

Desired Qualifications & Experience

  • At least three years experience in human services, preferably working with populations who are experiencing homelessness, mental health disorders, chronic health conditions, and/or substance use disorders
  • Capacity to respond to crisis situations with effective use of de-escalation skills
  • Strong communication skills – both verbal and written
  • Strong problem-solving skills
  • Strong documentation skills; HMIS experience preferred
  • Ability to collaborate and maintain community partnerships
  • A valid driver's license and reliable transportation is required.

Starting Wage: $49,000
