About Job

The Cardiac Care Manager is responsible for coordinating care for a complex, chronically ill cardiac patient population. This role focuses on assessing acuity, monitoring length of stay (LOS), managing throughput, and leading discharge planning for patients requiring home care services, physical and occupational therapy, and home infusion services—including heart failure patients being discharged on inotropic therapy.

Key Responsibilities:

  • Assess patient acuity and evaluate care needs throughout the hospital stay.

  • Monitor and manage LOS and patient throughput to support timely and appropriate care transitions.

  • Develop and implement comprehensive discharge plans in collaboration with the interdisciplinary team.

  • Coordinate post-discharge services including home care, PT/OT, and home infusion.

  • Support safe and effective discharges for heart failure patients requiring inotropic therapy at home.

  • Work closely with physicians, nurses, social workers, therapists, and other team members to ensure continuity of care.

  • Identify, capture, and trend delays in care and discharge; escalate and address barriers to patient flow.

  • Actively participate in rounds and case conferences to advocate for patient needs and discharge planning goals.

  • Contribute to performance improvement efforts related to throughput and discharge efficiency.

Preferred Criteria

  • Minimum of 3 years clinical experience in cardiac care or care management preferred.

  • Experience with discharge planning and coordination of complex post-acute services strongly preferred.

Required Criteria

  • Bachelors degree in Nursing (BSN)

  • Current NYS Registered Nurse (RN) licensure

  • 3 to 5 years of health care experience in an acute care setting

  • Superior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical
