About Job

RiseWell Community Services is seeking a compassionate full-time/ LMSW or LMHC to join our Home-BEST Program through Nassau County SPOA (Single Point of Access) services. Candidates can be hired without their license as long as they are working towards obtaining licensure. This is a clinical role and will count towards clinical hours for LCSW. In this role, the LMSW or LMHC will work with high risk children/ adolescents age 5-21 in their home and community to provide extra supports to prevent hospitalizations or residential placement.

Our Care Coordination teams help individuals with serious mental illness, chronic medical conditions, or substance abuse disorders. We connect participants with behavioral health specialists, primary medical care, housing services, vocational support, and entitlements. Our goal is to make sure the participant stays on top of appointments and receives the vital care needed for a healthy life.

Qualifications:
Driver’s License (Required)
LMSW/ LMHC (Preferred)
Master’s (Required)

SALARY: $27.47- $28.03 per hour
