Location: West Hempstead, NY | Schedule: Full time 35/hours | Status: Non-Exempt |Salary Range: $58,000-60,000/Annually
The Opportunity
Family and Children's Association (FCA) is seeking a dedicated Social Worker to join our team at Lakeview House, a community residence for adolescents with emotional impairments. This role supports youth who require a structured environment to achieve successful functioning and integration back into their families, communities, or independent living.
The Social Worker will ensure the effective and efficient delivery of services in compliance with OMH guidelines and agency policies and procedures. This position offers the opportunity to lead, grow, and make a meaningful impact in the lives of adolescents.
Responsibilities
  • Provide direct counseling, crisis intervention, and manage treatment plans for residents.
  • Conduct family engagement events and facilitate reunification preparation and workshops.
  • Complete all required documentation, including Medicaid billable notes, incident reports, and regulatory filings.
  • Develop and implement group curricula in collaboration with the Program Coordinator.
  • Coordinate screenings, intakes, and enrollment of new residents.
  • Communicate and collaborate with outside providers and school districts regarding clinical and educational plans.
  • Lead bi-weekly treatment team meetings and coordinate discharge planning and aftercare follow-up for a minimum of three months post-discharge.
  • Provide referrals and linkages to external agencies to support residents and their families.
Qualifications
  • Master's degree required; professional licensure preferred.
  • Minimum of two years' experience working with adolescents or in a community residential setting.
  • Proficiency in Microsoft Office applications is required.
  • Valid New York State driver's license with a demonstrated record of safe and responsible driving.
