- Facilitate referrals for chronic disease self-management education in both individual and group formats, promoting increased understanding and engagement in long-term health improvement strategies.
- Assist uninsured participants by linking them to appropriate community-based health and wellness programs to address gaps in care.
- Identify and refer participants to accessible community locations that provide educational opportunities and internet access to support their ongoing resource needs.
- Educate individuals on the significance of substance use treatment and connect them with appropriate community-based support services.
- Provide one-on-one support to help participants access accurate health information and effectively navigate complex healthcare and social service systems.
- Deliver limited case management services for detainees enrolled in the program, including support in understanding and adhering to their chronic disease treatment plans.
- Conduct culturally competent health and substance use education in accordance with agency protocols, focusing on chronic disease prevention, physical activity, and nutrition. Maintain accurate documentation of all patient-related activities in compliance with established procedures and protocols.
- Knowledge of the social, economic, and environmental conditions of clients served, characteristics of behavior of mental health and substance use, and available community resources
- Knowledge of community resources
- Skilled in the use of interpersonal skills
- Ability to relate to clientele in a supportive and therapeutic manner; to follow written and verbal directions; to record basic data
- Ability to prepare a variety of reports and documents
- Ability to effectively use computer applications to collect, organize and present data; ability to communicate effectively in oral and written forms
- Ability to build and maintain cooperative and effective relationships with clientele, co-workers, and resource staff
- Ability to work independently and with a team
