Position: Medical Social Worker

Based in Albuquerque, NM, our mission is to provide compassionate, personalized end-of-life care that honors every patient’s dignity and supports their families through one of life’s most profound transitions.We are currently seeking a Part-Time Medical Social Worker (MSW) to join our patient-centered hospice team. If you have a heart for helping others navigate the complexities of end-of-life care and are looking to make a lasting impact in your community, we would love to hear from you.Position SummaryAs a Hospice Medical Social Worker, you will provide critical psychosocial support to patients and families, helping them cope with illness, grief, and end-of-life decision-making. You will serve as a vital bridge between the hospice team, the patient, and the community—ensuring that emotional, financial, and resource needs are met with compassion and expertise.Key Responsibilities
  • Accept and respond to referrals from the interdisciplinary team for patients requiring social work services
  • Conduct assessments to evaluate patients’ psychological, emotional, social, and economic needs
  • Plan and provide short-term, goal-oriented counseling and support to patients and their loved ones
  • Connect patients and families to appropriate community resources and support services
  • Maintain accurate and timely documentation in accordance with the Plan of Care and hospice policies
  • Participate in interdisciplinary case conferences and provide psychosocial input to care plans
  • Make scheduled home visits and participate in on-call rotation as needed
  • Uphold patient rights and dignity, promote comfort, and support end-of-life transitions with empathy and professionalism
What You Bring
  • Education:
  • Master’s degree in Social Work (MSW) from a school accredited by the Council on Social Work Education (CSWE)
  • Experience:
  • Minimum 1 year of social work experience, preferably in a healthcare setting
  • Licensure & Certification:
  • Current license to practice as a Social Worker in the state of New Mexico
  • CPR certification
  • Valid driver's license with current auto liability insurance
  • Skills & Qualities:
  • Strong interpersonal and counseling skills
  • Excellent communication, documentation, and time management skills
  • Knowledge of Medicare/Medicaid and community resources
  • Ability to work independently in home settings
  • Professionalism, empathy, and emotional resilience
