Licensed Clinical Social Worker, Hospice

The Licensed Clinical Social Worker (LCSW) provides emotional, psychosocial, and practical support to patients and families facing end-of-life issues.

Working as a vital member of the interdisciplinary hospice team, the LCSW ensures that patients receive compassionate care in alignment with their values and preferences while helping families navigate anticipatory grief, loss, and community resources.

Position Responsibilities:

  • Conduct psychosocial assessments for hospice patients and families within 5 days of admission.
  • Develop and implement individualized care plans addressing social, emotional, and financial needs.
  • Provide counseling for patients, caregivers, and families dealing with terminal illness, grief, loss, and end-of-life transitions.
  • Participate in interdisciplinary team meetings to coordinate and update care plans.
  • Advocate for patient rights and ensure cultural, spiritual, and personal values are respected.
  • Assist families with advanced directives, legal paperwork, funeral planning, and access to community resources.
  • Facilitate family meetings and provide crisis intervention when needed.
  • Document all assessments, care plans, visits, and interventions in accordance with agency and regulatory standards.
  • Collaborate with other providers, including chaplains, nurses, and physicians, to ensure holistic care.

Required Skills/Qualifications:

  • Master’s Degree in Social Work (MSW) from an accredited program.
  • Active LCSW license in the state.
  • Minimum of 1 year of experience in hospice or palliative care.
  • Knowledge of Medicare hospice regulations and end-of-life care principles.

Job Type: Full-time

Pay: $95,000.00 - $105,000.00 per year
