About Job

Provide direct services to clients, including linkage, advocating, supporting, and providing education as needed.

  • Prepare written individual service plan, incorporating input from clients, involved service providers, and clearly identifying problem areas and needs, strategies, and service objectives.
  • Respond to crisis situations within twenty-four hours.
  • Link clients with external programs or services, such as health services, recreational activities, childcare services, financial resources, employment resources, childcare, AA/NA groups, transportation resources, and others
  • Collaborate and coordinate with other service providers and professionals, such as attorneys, social workers, and probation/parole officers.
  • Complete all progress notes and quarterly reports in a timely fashion.
  • Recommend appropriate services for client.
  • Assure client is informed of his/her human rights.
  • Maintain weekly contact with referring agency.
  • Conduct discharge planning in accordance with program description.
  • Attend all meetings as required.
  • Provide accurate time sheets.
  • Advise Program Manager of the state of clinical services, its staff, and its needs and solicits his assistance in implementing solutions to problems.

Must hold at minimum a Bachelor’s degree in Health and Human Services to include social work, gerontology, psychology, psychiatric rehabilitation, special education, sociology, counseling, vocational rehabilitation, criminal justice and human services counseling, or a related field from an accredited college or university and experience providing mental health services and case management to individuals suffering from a mental illness is preferred. This position also requires a valid driver’s license, automobile and telecommunications availability, excellent verbal and written skills; counseling and social work theories and techniques; and community resources. Strong writing and case conceptualization skills and reference to a methodological. The successful completion of a criminal history or background investigation is required.

Pay: $21.00 - $26.00 per hour
