???? Rotation between Appleton & Neenah ED | ⏰ 10-hour shifts (9a–7:30p) | ???? Every 5th weekend rotation

Looking to make a real difference? We’re seeking a compassionate, skilled Social Worker MSW to provide top-notch psychosocial care and collaborate closely with our medical team — all while living the social work code of ethics.

???? Key Accountabilities

✔ Identify patients with complex care needs & build a trusting rapport with patients and families

✔ Conduct thorough psychosocial assessments and help shape care plans

✔ Communicate effectively with the healthcare team to enhance quality care

✔ Create psychosocial intervention plans considering emotional, financial & spiritual needs

✔ Advocate for patients, facilitate access to vital resources (meds, housing, transport, etc.)

✔ Coordinate smooth care transitions and support discharge planning

✔ Facilitate advanced care planning and, in some cases, lead psychoeducational support groups

???? Qualifications

Master’s degree in Social Work

Licensed/Certified: LCSW, APSW, CISW, or CSW

BLS certification (American Heart Association)

???? Physical Demands

Move freely (stand, stoop, bend, walk, etc.)

Lift up to 25 lbs

Exposure to blood and bodily fluids

???? Work Environment Highlights

Occasional exposure to sharp instruments and mechanical parts

Potential contact with hazardous materials and communicable diseases

Sometimes a noisy, always dynamic healthcare setting
