EA Family Services is hiring a full-time Lead Care Manager for our Tracy office, which covers the Central California area. This position would entail working with a dynamic team providing care coordination/case management for Medi-Cal enrollees. Must love working with people; have a respectful and objective attitude; be friendly and outgoing. Must have a BA/BS in the social services or equivalent experience. Looking for a Bilingual speaker who speaks both English/Spanish and has experience in the Community Supports Housing Program. The hourly range is $22.00 -$24.50 DOE. Great benefits. Must pass DOJ/FBI/CAIC clearances.
The position will offer a hybrid setting with in-office, remote, and in-field work.
Additional tasks will focus on coordinating services like CS Deposit support, which will require coordinating move-in costs and household item purchases.
Lead Care Manager responsibilities include:
- Coordinating with individuals and/or entities to ensure a seamless experience for the member and nonduplication of services.
- Engage eligible members.
- Oversee provision of ECM and CS services, including the completion of the assessment, development of a care plan, implementation of the care plan and creation of the housing plan.
- Offer services where the member lives, seeks care, or finds most easily accessible and within managed care plan (MCP) guidelines
- Connect members to other social services and supports they may need, including transportation and Community Supports.
- Advocate on behalf of members with healthcare professionals.
- Use motivational interviewing, trauma- informed care, and harm-reduction approaches.
- Coordinate with hospital staff on discharge plan.
- Accompany members on office visits, as needed and according to MCP guidelines.
- Monitor treatment adherence (including medication).
- Provide health promotion and self- management training.
- Provide housing navigation services by connecting Member’s to housing resources.
Lead Care Manager trainings include:
- Enhanced Care Management and Community Supports Services.
- Methods and processes for data sharing.
- Member rights and responsibilities.
- Clinical protocols, evidence-based practice guidelines.
- Population of Focus specific health needs and cultural awareness and sensitivity.
- Patient-centered care.
