Are you a dynamic RN with a passion for patient-centered transitional care? Advanced Health Care of Aurora is hiring a full-time Discharge Coordinator Nurse Manager — also referred to as a Transition Nurse Manager — to lead and streamline the discharge planning process for our short-term skilled nursing patients.

Key Responsibilities:

• Collaborate with patients and families to coordinate discharge planning and next steps.
• Liaise with hospitals, rehab teams, home health providers, and others to ensure smooth transitions.
• Manage clerical responsibilities related to discharges including scheduling, data entry, and documentation.
• Handle incoming calls and provide facility tours as needed.
• Maintain strong communication across interdisciplinary teams.
• Provide exceptional customer service and support to all stakeholders.

• Current RN license in Colorado
• Strong organizational and time management abilities
• Excellent interpersonal, communication, and customer service skills
• Proficiency with basic computer systems, phone systems, and clerical tools

Full-time

• 4 x10-hr shifts OR
• 5 x8- hr shifts
