Exodus Recovery, Inc. (Exodus) is looking to hire individuals to initiate contacts with a variety of community resources; and implement linkages and discharge planning for clients in the Exodus Recovery Urgent Care Centers. The Discharge Planner shall meet the qualifications as indicated in the Short-Doyle Medi-Cal Rehab Option Manual and provide services in accordance with the scope of their practice.

Requirements:

• Master’s Degree preferred.
• Experience with case management with exposure to populations of Chemical dependency and mental health and working within interdisciplinary team and outside community agencies is highly desired.
• Strong verbal and written communication skills.
• Strong knowledge of community resources. Ability to work independently and shows mature judgment.

EDUCATION:

• Master’s degree preferred in a mental health related field; RN, PhD, PsyD, LCSW, MFT preferred.
• Licensed eligible desirable.
• Current CPR Certification and current CPI Certification preferred.
