Social Worker - independence, autonomy, and some flexibility

Care for the social and emotional needs of the aging population through end of life. Make a lasting difference in patients and families lives. Collaborate with the hospice interdisciplinary team. Counsel patients and families on stress management. Provide crisis intervention. Design and implement patient treatment plans. Attend regularly scheduled interdisciplinary group meetings. Assist the team in recognizing the psychological stress of a terminal illness. Identify and connect patients with community resources.

What you will need:

· Active license in good standing with the state of Idaho

· BSW required, MSW preferred

· CPR certification, we will provide if needed

· Positive attitude
