About Job

The Asthma Care Coordinator works within a multi-disciplinary health care team in the delivery of comprehensive services for patients diagnosed with asthma or COPD in a primary care setting. Acting as the primary contact person for the family, the Asthma Care Coordinator assists the health care team in the development and implementation of a health care plan tailored to the needs of the client and the client’s family in order to promote continuity of care, improves referral tracking, and ultimately reduces the rate and severity of disease-related complications

Reports To: Program Director, Quality and Health Education

Qualifications:

1. Must have a High School Diploma or General Educational Development (GED) certificate. While this position is considered an entry-level position, experience in practical nursing, preferably in an ambulatory care facility or a health education background in chronic disease is strongly desirable.

2. Possess the ability to work independently and as team member.

3. Possess excellent organizational and problem-solving skills and can prioritize.

4. Computer skills in Microsoft Office programs (Word, Excel, etc.), and database systems.

5. Possess the ability to perform a variety of tasks after training.

6. Demonstrated ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles.

8. Fluency in English (speaks, read, and write).

9. Fluency in Spanish (speaks, translate, read, and write) is desired.

10. Ability to maintain absolute confidentiality about health care and another patient/client information.

11. Current California driver’s license, appropriate insurance coverage and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
