- Processes all incoming patient referrals from physicians and acute/non-acute facilities.
- Screens referrals for admission to home health services based on agency policies and procedures.
- Verifies Medicare benefit eligibility through on-line DDE checks; and if necessary, confirm beneficiary’s status with other agencies by phone.
- Communicates medical complexity/acuity of all referred patients and their orders to the Clinical Coordinator.
- Coordinates patient care with various members of the healthcare team, including physicians, physician’s office staff, hospital discharge planners/case managers, and Territory Managers.
- Currently licensed as a Registered Nurse or Licensed Practical Nurse in Florida is preferred.
- At least one (1) year home health care experience preferred with current knowledge of CMS Clinical and Reimbursement Guidelines and COP.
- Able to work autonomously with minimum supervision.
- Possess organizational, problem-solving, time management, and decision-making skills.
- Advanced interpersonal skills including verbal and written communication skills.
