Job Summary

Baptist Medical Center is seeking a Licensed Clinical Social Worker (LCSW) in our Inpatient Behavioral Health unit at Baptist Jacksonville. This is a PRN Weekend shift opportunity.

As a Licensed Clinical Social Worker (LCSW), you will assess psychosocial needs and provide services that enhance patient functioning, emotional well-being and continued access to services and resources in the community. This LCSW will provide care coordination, arrange home health and DME, provide counseling, assists with placement and transfers, provides referrals to community services. Overall this LCSW contributes to the care of patients by addressing their social and emotional needs, promiting well being, and facilitating their adjustment to illness or injury.

As a Licensed Clinical Social Worker (LCSW), you will be responsible for:

• Assess psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community.
• Facilitates group and individual therapies on an inpatient behavioral health unit
• Completes documentation as required for regulatory purposes (Baker Act, clinical documentation)
• Assists with safety planning
