Care Transitions Nurse Manager – RN

HealthCare Support is actively seeking a Care Transitions Nurse Manager – RN to fill an opening with a prestigious Healthcare company in Brooklyn, NY.

Daily Responsibilities for Care Transitions Nurse Manager – RN :

• Will be making field visits but would not have their own caseload.
• Assisting and overseeing some of the more complex cases – act as a SME for Community Health Nurses in the field
• Communicate with interdisciplinary teams, social workers, and dietitians, helping coordinate that care
• Monday – Friday 8:30 am – 5:30 pm I On-call required
• This is a hybrid position with onsite & in-field component
  
  
  

Required Qualifications for Care Transitions Nurse Manager – RN :

• Active NY Registered Nursing license
• 3+ years’ experience as a Registered Nurse / RN
• 2+ years of recent experience at a Certified home health agency (CHHA)
• 1+ year of recent supervisory experience

Pay Details: $135,000 – $140,000 annually
