About Job

1. Provide for the medical and behavioral health related social services to assist resident in maintaining or improving their ability to manage their everyday physical, mental and psychosocial needs.

2. Perform assigned tasks, functions, and responsibilities in a professional, ethical, and diligent manner in accordance with state and federal laws, regulations, rules, and policies as well as home medical standards, policies, and procedures which include compliance with the requirements of Title 22, California Code of Regulations (CCR) for long term care.

3. Participate as member of the Inter-disciplinary team in developing, implementing or modifying treatment / care plans, level of care considerations, discharge planning and community referrals etc.for the resident with families or responsible parties. Provide counseling or psychotherapy for residents with psychological and/or emotional problems in accordance with established guidelines.

4. Provide observation, ongoing assessment and therapeutic intervention consistent with resident’s physical and psychological status, Evaluate effectiveness of resident’s treatment and record progress.

5. Participate with the Admission Team Process, reviewing applications for prospective resident and conducting Pre-Admission Appraisals.

6. Participate in the development of treatment/care plan, initial assessment, level of care considerations, and discharge planning.

7. Identify factors, which negatively impact the resident’s physical, mental and psychosocial wellbeing. Assess and refer residents with alcohol related issues. Identify other influential conditions such as depression, family conflict, grief & loss, adjustment, interpersonal problems, chronic or acute pain, and physical or chemical restraints.

8. Be aware of resident’s rights, identifying the potential for resident abuse, report promptly to treatment team members, quality control, and administrators about any adverse behavior toward resident by staff, family member, fiduciary or responsible party.

9. Report and document all incident of physical, emotional, sexual or fiduciary abuse. Assist families with end of life planning. Participate as liaison between Veterans home of Yountville, resident, families and responsible parties.

10. Organize, maintain and provide necessary health care services consistent with policies and procedures.

11. Prepare, maintain, record and follow up on resident's treatment/care plan with the resident in a timely manner as required by Veterans Home of Yountville. Provide oral and/or written reports.

12. Provide individual and group therapy for resident in order to achieve, identified therapeutic goals.

13. Prepare documentation of Social History Evaluations (SHE) and Minimum Data Sheet (MDS) in a timely manner in accordance with Medicare and Title-22 regulations.

14. Act as an advocate for resident to ensure appropriate treatment is provided and grievance policies and procedures are followed, Assist with Advance Directives, Power of Attorney and Assistance Payee service’s paper work.

15. Given the circumstance and environment all duties/ responsibilities assigned to the contractor/provider are subject to change and modification.

16. Provider shall wear an identification badge which includes the name of the Contractor, the Provider’s name, and a photo ID. This badge shall be worn at all times while on grounds of the Veteran’s Home.

17. Provider shall ensure resident medical record information is maintained as “CONFIDENTIAL” and shall be held in trust and confidence. Information shall be used only for the purposes contemplated under this contract.

18. Personal Protective Equipment (PPE) a. All LCSWs as provided by the Contractor must use PPE (gloves, face-shields, masks, etc.) while providing services as directed by the Contract Manager.

b. Contractor staff may obtain and use PPE from the Home for services on an as needed basis at no charge upon approval from the Contract Manager.

19. Contractor must have a minimum number of Providers to adequately provide services who meet the requirements specified in providers' responsibilities.

20. The Contractor shall ensure each Provider has an identification badge which includes the name of the Provider, Name of the Contractor (if different), and a photo identification. The badge shall be worn by the Provider at all times while at the Home.

21. In the event any Providers provided by the Contractor are dismissed or declined at a home, the Contractor shall inform the Contract Manager or Designee at any other Home of which the said Provider provides service of this fact upon notification of the dismissal

Pay: $47.00 per hour
