Queen City Hospice is seeking a Hospice Social Work Practitioner to join our compassionate team of professional clinicians dedicated to making a difference in the lives of patients and families on their end-of-life journey. Shift Information: Monday-Friday 8a-5p with some weekend and evening on-call responsibilities.

Key Responsibilities:
- Conduct psychosocial assessments of patients and their families/caregivers to identify needs effectively, ensuring they are addressed in the patient’s plan of care.
- Provide exceptional service to patients, families, and team members, supporting everyone involved in the care process.
- Offer counseling to patients and families regarding stress and coping difficulties.
- Ensure accurate and timely documentation of patient services in accordance with care plans.
- Assist families with applications for Supplemental Security Income, Hospice services, Medicaid, and Medicaid Waiver, and connect them with community resources.
- Act as a liaison between patients, families, and various community agencies.
Qualifications:
- Degree in Social Work; a Bachelor's degree is required, while a Master’s degree is preferred.
- Current State Licensure in Social Work (LSW, LISW, or LPSW-S in Ohio).
- At least one year of experience in a healthcare or hospice setting.
- A valid driver's license and proof of insurance is mandatory.
