Summary
In conjunction with nursing leadership, is responsible for setting resident-directed quality care and quality-of-life standards in the community as well as evaluating and implementing person-directed policies and procedures. Evaluates clinical and interdisciplinary resident care, and makes recommendations based on current regulatory and best practice standards by performing the following duties.
Essential Duties And Responsibilities
Registered Nurse (RN) licensed in the State of Colorado in good standing. Five to ten years’ related experience and/or training and at least two years of nursing leadership experience in sub-acute or long-term care.
CERTIFICATES, LICENSES, REGISTRATIONS
Registered Nurse licensed in the State of Colorado in good standing. Current and active Certified Pulmonary Resuscitation (CPR) certification.

Summary
In conjunction with nursing leadership, is responsible for setting resident-directed quality care and quality-of-life standards in the community as well as evaluating and implementing person-directed policies and procedures. Evaluates clinical and interdisciplinary resident care, and makes recommendations based on current regulatory and best practice standards by performing the following duties.
Essential Duties And Responsibilities
- Evaluates the extent person-directed care practices exist on community teams and addresses barriers. * Coordinates rounds to identify and address quality-of-care issues. Identifies and addresses loneliness, helplessness, and boredom.
- Maintains and shares a working knowledge of regulations, Shalom Cares’ policies, and standards of care in order to elevate the quality of care provided
- Utilizes learning circles to build the community team; problem solves complex medical/ psychosocial situations related to resident choice and risk
- Utilizes the RAI/MDS process for person-directed assessment, care planning resident stories, and regulatory compliance
- Evaluates and implements nursing policies and procedures that reflect person-directed care and evidence based practices
- Models, teaches and utilizes effective communication skills such as active listening, giving meaningful feedback, communicating ideas clearly, dealing with emotional behavior, conflict resolution, and understanding the role of diversity in communication; demonstrates the importance of leadership by example
- Facilitates the staffing process to promote team-based staffing, self-directed community team development as well as develop nurses who effectively provide leadership and support
- Participates in an effective quality assurance performance improvement (QAPI) process that meets regulatory requirements and results in specific action plans when issues are identified; identifies trends and coordinates community-based performance improvement plans as needed
- Takes responsibility not only for own education, leadership, and personal development, but also develops a culture where staff and resident personal growth is a part of community life
- Identifies staff educational needs and works with the DON/Clinical Administrator to coordinate education; Conducts care plan training, tracking and auditing
- Understands the community/nursing budget and teaches fiscal management to direct care staff
- Ensures sufficient resources (i.e., central supply, equipment, technology, trained staff) are present to provide quality care and promote quality of life
- Works closely with the Admissions Coordinator to provide a timely, person-directed admissions process; evaluates the process to verify resident and family preferences, fears, and needs are promptly identified and addressed
- Coordinates nursing, dietary, social services, and activity components of resident care
- Evaluates and responds to family education needs when they arise to include personal contact with family members to address end-of-life issues
- Establishes consistent, quality clinical services in all levels of care
- Audits MDS, care plans, and related paperwork done by nursing staff; enters MDS data as necessary
- Maintains the clinical aspects of the floor in accordance with Federal and State regulations; acts as an alternate in the absence of assigned nurse/aide to ensure duties and cares are not interrupted
- Investigates abuse allegations; documents and assists in process of occurrence reporting
- Attends committee meetings as assigned or required
- Assists in maintaining HCFA reports
- Documents incident reports and follow-up
- Promotes and facilitates infection control policies and provides in-services as required; follows up on trends and patterns as identified by infection control reports
- Coordinates and facilitates the administration of resident immunizations; administers and tracks resident PPD, pneumococcal and flu immunizations
- Coordinates management and documentation of resident skin condition and wound care interventions; provides education and feedback to staff and team leaders about wound management and documentation
- Takes call and provides off-hour leadership support as necessary
- Other duties may be assigned
