Job Summary
We are seeking a dedicated and compassionate Social Worker to join our team. The ideal candidate will have experience working with residents, staff and families. This role involves providing support, guidance, and advocacy to residents while ensuring compliance with relevant regulations such as HIPAA. The Social Worker will be responsible for developing care plans, conducting assessments, and facilitating access to resources that promote the well-being of clients.
Responsibilities
- Conduct comprehensive assessments to identify residents needs and develop individualized care plans.
- Maintain accurate medical documentation and utilize medical terminology in all reports.
- Collaborate with interdisciplinary teams to coordinate services for clients, ensuring holistic care.
- Advocate for clients’ rights and access to necessary resources within the community.
- Engage in ongoing professional development related to special education and geriatric care practices.
- Ensure compliance with HIPAA regulations in all aspects of client interaction and documentation.
Experience
- A degree in Social Work or a related field is preferred or a completed Social Service Designee course.
- Experience working in the Long and Short term Skilled Nursing environment.
- Knowledge of medical documentation processes and terminology is essential.
- Previous experience in geriatric care or working with older adults is beneficial.
- Strong communication skills, both verbal and written, are necessary for effective client interaction.
- Ability to work collaboratively within a team-oriented environment while maintaining client confidentiality.
