Vita Blue Home Care is seeking a compassionate, organized, and detail-oriented Care Coordinator to join our administrative support team. In this role, you will work closely with patients, caregivers, and team members to ensure smooth care delivery and patient satisfaction. This is a full-time, in-office role focused on care coordination—not a field or clinical position.
Key Responsibilities:
- Develop, monitor, and evaluate individualized home care plans to ensure quality and compliance
- Coordinate and manage daily schedules for home health aides, ensuring coverage for all assigned shifts
- Respond to last-minute call-outs and secure appropriate replacements to prevent service disruption
- Build and maintain a pool of on-call aides in partnership with Human Resources
- Serve as a liaison with referral sources, case managers, and healthcare professionals to deliver exceptional service
- Match caregivers to clients based on compatibility, care needs, and availability
- Manage a caseload of 100+ patients while ensuring consistent follow-up and service quality
- Review and explain care plans with patients and caregivers to ensure understanding and adherence
- Address and resolve caregiver and client concerns, grievances, and complaints in a timely and professional manner
- Monitor utilization of authorized hours and address overuse as needed
- Report and document performance or conduct issues among personnel
- Maintain high levels of client satisfaction and ensure caseload retention
- Collaborate with the internal care team to accomplish agency goals and improve service deliver.
Minimum Qualifications:
- High school graduate
- 2+ Years experience in a Home Care or Service Coordination role.
- Demonstrated capability maintaining strict confidentiality
- Proven ability to generate leads and monitor referrals
- Strong typing and computer skills
- Comfortable with closing/asking for business
- Well organized, accurate, and attentive to detail
- Excellent communication, public relations and follow up skills
- Experience with HHA Exchange and EVV
- Bilingual English/Spanish preferred.
