The Position:

We're looking for an experienced and passionate Discharge Planner (RN) to coordinate discharge planning with the resident/guest’s attending provider and assist in contacting appropriate community resources. Responsibilities include:

  • Coordinates and manages discharge planning activities in compliance with all federal and state regulations.
  • Meets with resident/guest and/or family within 72 hours of admission to obtain resident/guest/family goals for discharge.
  • Collaborate with healthcare team, including providers, licensed nurses and social workers.
  • Facilitate resident/guest referrals to external facilities/providers that are needed for continuation of care.
  • Assist in arranging timely home health services and equipment for discharge.


Requirements:

  • Graduate of an accredited school of nursing.
  • Current license as a Registered Nurse in the State of Hawaii required.
  • 4 or more years’ experience in restorative and gerontological nursing.
  • Discharge planning experience, preferred.
