The ideal candidate will be results-driven, team-and detail-oriented professional who is comfortable working on multiple tasks in a deadline-oriented environment.
Responsibilities:
- Provide mental health and substance abuse screening, assessment, orientation, treatment and follow-up services primarily within the agency, but also in other settings as needed.
- Provide individual, couples, family or group counseling within the scope of one's competencies.
- Provide crisis intervention.
- Provide referral consultation and education to the community regarding issues of chemical dependency and mental health.
- Provide chemical dependency and mental health consultation, education and training to agency personnel.
- Develop and implement treatment plans and document services consistent with Thrive standards and/or insurance requirements.
- Collaborate with other providers, agencies and individuals in the client's network of care.
- Provide relapse prevention, discharge planning, follow-up and aftercare.
- Provide education, training, and modeling in basic daily living skills, social or parenting skills.
- Teach clients effective techniques for managing their medication as prescribed.
- Coordinate services within Thrive as needed.
- Assist clients in developing and strengthening their support network to accomplish goals and objectives.
- Produce, complete, and maintain billing forms, logs, data collection forms, clinical documentation and client records in accordance with Thrive standards and state and federal regulations.
- Assist clients in maintaining appropriate medical coverage at all times. Collaborate with Case Manager in doing so when appropriate.
- Schedule and maintain client hours sufficient to achieve individual / program billing targets.
- Master's Degree in the human service field of social work or counseling preferred.
- Minimum (2) year experience working with clients with disabilities preferred.
- Experience or training in related state and federal assistance programs also preferred.
- Licensed at an independent level
- Basic computer literacy skills are necessary
- Valid driver's license and automobile insurance which meets Rhode Island minimum standards law, or the ability to obtain necessary transportation in order to perform the job responsibilities/tasks is necessary for community based positions.
