Summary

The Social Worker, Program Coordinator for Home-Based Primary Care (HBPC) is a Social Worker aligned under the Associate Chief Nurse for Geriatric and Extended Care and manages HBPC, Medical Foster Home (MFH), and Veteran Directed Care (VDC).

Duties

Major Duties include but are not limited to:

  • Responsible for assessment, planning, and provision of case management services in HBPC.
  • Accountable for the management, supervision and evaluation of care for the HBPC program, MFH and VDC.
  • Supervises the clinical staff aligned with HBPC, to include orientation, performance evaluations, competency assessments, and compliance with VA guidelines and recurring education requirements.
  • Facilitates an atmosphere of interactive management and development of collegial relationships among interdisciplinary team members, serving as a link between services where staff are matrixed to HBPC, and all healthcare disciplines included on the HBPC teams.
  • Responsible for implementation of the vision, mission, philosophy, and core values for the organization.
  • responsible for directing the clinical and administrative services offered by programs to ensure compliance with local and VHA standards and policies, as well as accreditation standards for home care organizations.

  • Responsible for the development and continued effective functioning of the interdisciplinary health care team, recognizing and utilizing the skills, knowledge, and contributions of each team member.
  • Collaborates with the HBPC Medical Director, Veteran Integrated Service Network (VISN) leadership, and VA Central office staff on a regular and ongoing basis.
  • Equipment Management: Completes inventory annually, evaluates need for additional equipment, and completes orders through VA software system. Updates Non-Critical Re-usable medical equipment manufacturer care update, annually.
  • Emergency Management: Manages emergencies or potential emergencies, delineating scope and breadth of emergency and creating a plan to ascertain status of those Veterans enrolled in HBPC, MFH and VDC. Will evaluate response for opportunities to improve the process.
  • Committees: Participates in assigned committees and completes quality monitors related to those committees such as infection control, PCS quarterly Committee meeting, Medical Records Committee etc. Participates in VISN committees and/or national committees and demonstrates the ability to accurately implement national polices and/or VISN policies at a facility level and measure outcomes to ensure stated goals are met.
  • Participates in National calls and VISN Community of Practice calls associated with services within his/her purview.
  • Human Resources: Oversees vacation schedule distribution and set parameters for number staff on leave at one time. Works with ERLR to address personnel issues. Updates and administers performance appraisals and annual promotional reviews. Works the Medical Director and Rehabilitation to administer credentialing process to include chart audits and review of scope of practice. Will evaluate and monitor staffing to ensure compliance with national recommendations.
  • Quality Assurance: Creates a surveillance plan to monitor program goals and staff performance. This includes, but is not limited to, infection control, EPRP data, VVC encounters, staff productivity etc.
  • Leadership: Coaches individual staff members to assist with attaining personal professional goals. Incumbent demonstrates leadership skills that facilitates and encourages staff to operate as a team by developing trust within the team, giving staff the ability to express and discuss concerns regarding current processes
  • HBPC/MFH/VDC: Works with nurse coordinator and staff, collaboratively, to resolve issues, improve processes/care, improve safety of Veterans and staff, advance technology used in care of our patients and improve customer service.
  • Performs other duties as assigned.

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going tohttp://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
  • English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations: Social Worker (Program Coordinator), GS-13
  • Experience and Education: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
  • Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
  • Demonstrated Knowledge, Skills, and Abilities (KSAs): In addition to the experience above, the candidate must demonstrate all of the following KSAs:
    • Skill in coordinating and implementing one or more specialty program(s), across the facility.
    • Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
    • Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
    • Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.
    • Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
    • Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.
    • Knowledge of organizational structure and impact on program operations.
Preferred Experience: Experience in Home Based Care or Primary Care with a leadership background
