A caregiver who excels in this role will:
- Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems.
- Perform psychosocial assessments and provide interventions to assist patients and families. This may include counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings.
- Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans.
- Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers.
- Maintain a network of resources for consultation and connect patients with community resources.
- Supervise social work interns and assist with training new caregivers.
- Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.
Minimum qualifications for the ideal future caregiver include:
- Master's degree in social work from a CSWE accredited program
- State Licensed Social Worker (LSW
- Two years of experience in a hospital or healthcare setting
- Licensed Independent Social Worker (LISW) within five years of hire

Physical Requirements:
- Manual dexterity to operate office equipment.
- May require periods of sitting, standing and the ability to walk to various locations throughout the Foundation to attend meetings.
- Must have normal or correction vision
- Must be able to clearly communicate verbally by phone.
