Compensation Range: $72/hr - 75/hr commensurate with experience

Overview

The Adult Day Health Care (ADHC) Program at Housing Works is a unique community-based health care program that provides comprehensive nursing, social work, nutrition, mental health and substance use services to individuals living with chronic illnesses including HIV, behavioral health conditions, diabetes, hypertension and others. Housing Works is seeking Case Manager to provide mental health/substance use/supportive counseling, crisis interventions, and case management services to clients of the Adult Day Health Care Program, located in Brooklyn. This is a unique opportunity for a Social Worker/Case Manager who is focused on patient centered community mental health and harm reduction model. With the support of the Director of Clinical Services, Clinical Supervisor and interdisciplinary team of professionals, the Licensed Master Social Worker will help achieve improved health outcomes for our client population. The ideal candidate will be familiar with evidence-based interventions and comfortable using data to guide practice. NYS Licensed Master Social Worker required. Bilingual Spanish preferred.

Responsibilities

  • Manages a caseload of 30 – 40 active clients, not inclusive of staff coverage.
  • Provides mental health/case management services including but not limited to monthly progress summaries, quarterly/bi-annual assessments, quarterly/bi-annual care planning, counseling, outreach, case conferences, advocacy, referrals (on-site & off-site) and discharge planning.
  • Completes initial assessment and quarterly/bi-annual reassessments on all assigned clients in a timely manner to develop individualized treatment goals for each client.
  • Establishes treatment goals and objectives with clients as part of the interdisciplinary team.
  • Ensure timely submission of clinical documentations to managed care organizations for uninterrupted services.
  • Facilitates up to 6 groups per week, not inclusive of staff coverage
  • Attends all training, supervision, staff meetings, care planning sessions, and daily morning rounds.
  • Provides crisis intervention services and, if necessary, refers clients for psychiatric evaluations and other emergency services.
  • Coordinates the provision of multidisciplinary services as outlined in interdisciplinary care plan.
  • Other duties as assigned by the Clinical Supervisor/Program or Vice President of Operations
  • Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS.
