Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Care Management team! Care Managers (CM) partner with licensed care management staff members and other care team members to deliver a comprehensive, high-quality, patient-centered experience. Care Managers conduct time-sensitive outreach and/or provide support which includes short- and long-term capacities to coordinate care for high-risk and 2 disease-management patients

*Compensation is based on each candidate's experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.

Main Duties & Responsibilities
  • Conducts outreach to complete between 90-120 health risk assessments per month.
  • Conducts successful outreach to 120 disease management eligible or enrolled per month.
  • Collaborates with Erie's Licensed Care Managers (LCMs), Transitions of Care (TOC)team, the Social Drivers of Health (SDOH), Health Promotions, and all other patient program team members to provide care coordination services and link with best fitprogram for patient needs.
  • Documents care team communication, resulting action steps, and activities in Erie's electronic health record (Epic) for all participating patients, aligned with Erie and CARES Act standards (e.g., no medical jargon, etc.).
  • Documents all activities in additional external system as required/appropriate.


Qualifications

Education

Required:
  • High School diploma or equivalent required


Preferred:
  • Community Health Worker training
  • Bachelor's degree


Experience
  • At least 1 year of experience in community-based organizations, safety net settings, or with vulnerable population required
  • One year of customer service experience, preferably in a health care setting preferred
  • Experience conducting patient assessments preferred.
  • Phone-based outreach experience preferred.
  • Experience identifying, referring to, and working with community-based organizations preferred.
  • Experience with health system navigation and patient education preferred.
  • Managed care, care management, or care coordination experience or exposure preferred.


Skills and Knowledge
  • Strong computer skills required: expert in Microsoft Office, Excel and ability to learn to expert level other electronic databases (Epic electronic medical record system, etc.).
  • Attentive to detail required
  • Strong interpersonal and oral communication skills required
  • Comfort using phone-based translation services required.
