Job Summary
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have a strong background in mental health counseling and a passion for helping individuals navigate their challenges. This role involves providing support to clients in various settings, including nursing homes and home community care, while utilizing therapeutic techniques such as psychotherapy and behavioral therapy. The Social Worker will be responsible for conducting patient assessments, developing treatment plans, and providing clinical counseling to those dealing with issues such as PTSD.

Responsibilities

  • Conduct comprehensive patient assessments to identify individual needs and develop appropriate treatment plans.
  • Provide psychotherapy and clinical counseling to clients in various settings, including nursing homes and community environments.
  • Implement evidence-based practices in behavioral therapy to support clients' mental health needs.
  • Offer diagnostic evaluations to determine the best course of action for client care.
  • Collaborate with healthcare professionals to ensure holistic care for clients.
  • Maintain accurate documentation of client progress and treatment outcomes.
  • Advocate for clients' needs within the healthcare system and connect them with necessary resources.
  • Provide education and support to clients and their families regarding mental health issues and coping strategies.

Requirements

  • A Master’s degree in Social Work (MSW) from an accredited institution is required.
  • Valid state licensure as a Social Worker (LCSW or equivalent).
  • Experience in mental health counseling, particularly with PTSD care, is preferred.
  • Familiarity with patient assessment techniques and clinical counseling practices.
  • Strong communication skills, both verbal and written, with the ability to build rapport with clients.
  • Ability to work collaboratively within a multidisciplinary team environment.
  • Knowledge of home community care services is advantageous.
  • Commitment to ongoing professional development in social work practices and methodologies.

Pay: $58,031.78 - $69,887.73 per year
