Minimum Hiring Wage: From $26.89 per hourJob Details: Scheduled Hours: 8:00am - 4:30pm, every 6th - 8th weekend rotationJob Description:This position will work in Social Work and Behavioral Health Assessment Team.Provide patients and their families, including vulnerable populations, with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services address their social, emotional, and financial needs. Services provided include clinical assessment, crisis intervention, brief therapy, case consultation with the healthcare team and collaboration with other professionals in the community. These services advise family caregivers, provide patient education and counseling, and make necessary referrals to community resources for continuity of care.Responsible for providing direct support to people with varying degrees of mental illnesses and other related issues. Utilize intervention strategies to assist with rapid stabilization, de-escalation and therapeutic mediation. Conduct a thorough, accurate, clinically sound Triage/Psychosocial comprehensive mental health screening for qualifying patients presenting to the Health System as well as via tele-medicine. Gather collateral information from family, other providers, etc. in order to provide a more thorough screening of immediate needs. Actively coordinate care, identify and address gaps in service needs and make appropriate recommendations.
