Key Responsibilities:
- Embrace Connected Communities’ Mission: Uphold our values, philosophy, and procedures to support healing and well-being in every interaction.
- Therapeutic Engagement: Manage a caseload of clients, offering weekly and bi-weekly sessions tailored to each individual's strengths and needs through evidence-based strategies.
- Comprehensive Assessment: Conduct detailed diagnostic assessments using a Biopsychosocial approach, identifying strengths, areas for growth, and considering family, social, educational, medical, and personal aspects of each client.
- Collaborative Treatment Planning: Develop and document clear, actionable treatment plans with client input, emphasizing measurable goals that support growth and well-being.
- Ensure Continuity of Care: Coordinate with other providers monthly, ensuring seamless, integrated care that meets the needs of each client.
- Professional Documentation: Maintain timely progress notes, treatment updates, and comply with audit requirements, guaranteeing clear, effective client documentation.
- Responsive Communication: Answer emails, calls, and messages within 24 hours, reflecting the reliability and accessibility our clients and team expect.
- Team Collaboration: Participate in both individual and group supervision, contributing to a supportive, growth-oriented clinical environment.
- Compliance & Professionalism: Complete required timesheets and incident reports promptly, adhering to organizational guidelines.
What You Bring:
- Credentials: Licensed or license-eligible in Virginia (LCSW, LPC) or Registered Resident in Counseling or Supervisee in Social Work.
- Educational Background: Master’s degree in a relevant human services field (Social Work, Psychology, Counseling).
- Professional Competencies: Strong organizational and time management skills, with the ability to manage confidential information within HIPAA guidelines.
- Communication Skills: Proficient in both written and oral communication, with a compassionate, relationship-building approach.
- Core Qualities: Integrity, empathy, and professionalism that shine through in your work and relationships.
- Therapeutic Knowledge: Foundational knowledge of therapeutic processes and clinical counseling skills, with a commitment to ongoing learning and development.

Pay: $30.00 - $35.00 per hour
