Job Title: Member Care Coordinator
Location: Chicago, IL (Full-time remote, with a few days of onsite training)
Job Type: Contract (6 Months)
Job Summary:
The Member Care Coordinator is responsible for conducting home health assessments, educating members about their care plans, and supporting clinical teams with non-clinical tasks. This role includes outreach, follow-ups, documentation, and coordination of services like transportation and appointments.
Key Responsibilities:
- Conduct in-home assessments and update records
- Perform member outreach and care plan follow-ups
- Educate and engage members using scripts
- Coordinate services and schedule appointments
- Support case management with data entry and communication
- Maintain HIPAA compliance and documentation accuracy
Requirements:
- Bachelors in Social Work/Psychology OR LVN/LPN/RN OR 3+ years in care coordination/managed care
- Knowledge of medical terminology and healthcare coordination
- Strong communication and customer service skills
- Proficient in Microsoft Office
- Valid driver’s license, reliable transportation, and willingness to travel
