Position: Social Worker

Department: Social Services

Reports To: Social Services Director

Overview:The Social Worker supports the psychosocial, emotional, and behavioral health needs of residents in a long-term care facility. This role requires exceptional organizational, communication, and multitasking skills, as well as proficiency in tools like Google Sheets, PointClickCare, and other facility systems. The Social Worker collaborates with residents, families, providers, and interdisciplinary teams to promote resident well-being and ensure their needs are met.

Hours: Monday - Friday 8:00am to 4:30pm

Key Responsibilities:

Behavior and Aggression Management:

  • Complete weekly GDR (Gradual Dose Reduction) tracking forms.
  • Maintain and update behavior tracking forms for all assigned residents.
  • Conduct vulnerability and aggression assessments pre- and post-admission, as well as ad hoc assessments for behavioral events, and weekly with the MDS

Resident Support and Social Services:

  • Identify residents’ strengths and social, emotional, and mental health needs through communication with residents, families, and staff.
  • Provide, develop, or assist residents in accessing services to meet identified needs.
  • Collaborate with the interdisciplinary team to promote resident rights and well-being.
  • Educate staff on the role of social services in supporting residents.

Care Planning and Coordination:

  • Participate in the development of written, interdisciplinary care plans to address social service needs.
  • Conduct behavior follow-ups and update care plans and interventions as required.
  • Complete all assigned MDS assessments, IDT (Interdisciplinary Team) care conferences, and prepare summaries, coordinate team completion.
  • Perform patient/resident discharge planning activities, including family education, documentation, and referrals to appropriate agencies.
  • Resident 5 Wishes Complete on admission and quarterly with the MDS
  • Discharge Planning and Discharge Review and Assessment

Resident and Family Engagement:

  • Hold meetings with residents, families, and significant others to provide support, information, and improve quality of life.
  • Lead one resident group session per week and participate in the Resident Buddy Program.
  • Round on all assigned residents a minimum of twice per shift to address concerns and provide support.

Administrative and Facility Support:

  • Complete admission paperwork with residents and families during admission to the facility, update admission paperwork as needed with new updates to the admission packet.
  • Label residents’ personal items and ensure proper handling for laundry services.
  • Deliver mail to residents and assist the front desk with delivering items as needed.
  • Answer phone calls and manage time to ensure front desk coverage. (cover the front desk when they go on lunch or to the bank, be familiar with resident money management systems)
  • Maintain accurate and timely documentation in Google Sheets, PointClickCare, and other systems.
  • Update GDR board, 15 min checks, wanderguard list, and Independent pass list and book daily, ensure all paperwork on the unit is relevant and up to date, MOST forms and books are up to date, vulnerability care tools, aggression care tools, and dementia care tool books are up-to-date.
  • Complete Appointment Packets and appointment follow-up spreadsheet, for residents for the following day, provide follow-up paperwork and instructions to the appropriate departments.
  • Ensure Elopement care plans and binders are up to date daily.
  • Ensure Bed hold policy is provided to residents who leave the facility.
  • Monitor residents leaving the facility and ensure the passbook is signed appropriately.
  • Amazon ordering for residents.
  • Assist with finding lost items for residents

Community Engagement and Referrals:

  • Establish and maintain relationships with community-based agencies and services.
  • Ensure referral flow and access to external resources to support resident needs.
  • Ensure residents' ancillary service needs are met.

Education and Staff Collaboration:

  • Train staff on behavioral interventions, mandatory training topics, and social service roles.
  • Build, develop, and maintain a work therapy program to enhance residents’ quality of life.

Additional Duties:

  • Assist with coordinating Resident Council and activities as needed.
  • Conduct audits and investigations as assigned by the Social Services Director.
  • Deliver timely updates to the Social Services Director and assist with any other tasks as assigned.
  • Assist and coordinate ancillary services needs for residents and support ancillary services when in the facility.
  • Assist with provider admission tours for potential new residents

Job Requirements:

  • Education: Bachelor’s Degree in Social Work (BSW), preferred.
  • Experience: Prior experience in long-term care, behavioral health, or related fields is highly desirable.

Skills:

  • Strong organizational and time management skills.
  • Exceptional verbal and written communication abilities.
  • Proficiency in Google Sheets, PointClickCare, and other facility software.
  • Ability to multitask, prioritize, and complete assignments efficiently.
  • Flexibility to support various facility functions and tasks.

Work Conditions:

  • Full-time position based in a long-term care facility.
  • Stationed at the nursing station with a laptop for daily operations.
  • Regular interaction with residents, families, staff, and interdisciplinary teams.
