Job Summary:

The Home Health Intake Coordinator is responsible for managing the intake process for home health patients, ensuring timely and accurate collection of referral information, verification of insurance eligibility, coordination of authorizations, and smooth transition into care. This position serves as a key liaison between referral sources, clinical staff, patients, and insurance providers to support the efficient delivery of home health services.

Key Responsibilities:

  • Receive and process home health referrals from hospitals, physicians, discharge planners, patients, and families.
  • Collect and verify complete patient demographics, insurance information, medical history, and physician orders.
  • Confirm eligibility and benefits for Medicare, Georgia Medicaid, and private insurance using appropriate verification systems (HETS, GAMMIS, clearinghouses).
  • Obtain prior authorizations when required by Medicare Advantage or private insurance plans.
  • Coordinate with clinical and administrative staff to schedule timely admissions and start-of-care visits.
  • Ensure all required documentation (physician orders, plans of care, face-to-face documentation) is complete and compliant with regulatory standards.
  • Communicate regularly with referral sources and update them on patient intake status.
  • Maintain accurate records in the Electronic Health Record (EHR) system.
  • Support billing and authorization staff by providing required intake and insurance documentation.
  • Provide excellent customer service to patients, families, referral partners, and internal teams.

Qualifications:

  • High School Diploma or equivalent required; Associate’s Degree preferred.
  • Minimum of 2 years of experience in home health, medical intake, or healthcare administrative support.
  • Strong knowledge of Medicare, Medicaid, and private insurance verification processes.
  • Familiarity with Georgia Medicaid (GAMMIS) and Medicare portals (HETS, MAC, etc.).
  • Experience with Electronic Health Records (EHR) systems preferred.
  • Excellent organizational, communication, and multitasking skills.
  • Ability to work efficiently in a fast-paced environment.
  • Customer service oriented with strong attention to detail.

Preferred Skills:

  • Prior experience as a home health intake coordinator is highly desirable.
  • Knowledge of home health billing processes and CMS regulations.
  • Proficiency in Microsoft Office Suite and insurance verification platforms.
