Overview: * Approves revisions to the plan of care of assigned patients.

Approves recertifications and discharges of assigned patients.
Ensures appropriate disciplines are utilized of assigned patients.
Monitors utilization of services including supplies
Monitors Lupas
Identifies and follows high risk patients
Performs Virtual Visits per Plan of Care
Facilitate Discharge Referrals
Makes recommendations for efficient and effective utilization of resources to achieve positive patient outcomes
Facilitates Case Conference and ensures patient’s goals are appropriate, meaningful and reachable
Ensures positive clinical as well as financial outcomes
Participates in the performance improvement process as directed
Monitors assigned patients data and make appropriate and timely interventions.
Adheres to established processes, policies and procedures
Participates in other organizational duties as needed

Qualifications:Graduate of an accredited school of nursing with a current NC license or RN multi-state licensure privilege through the Nurse Licensure Compact. Bachelor’s Degree in Nursing preferred. Two-three years of home health experience is required. Case Management experience required. Excellent working knowledge of OASIS and related outcomes required. Must have excellent clinical and critical thinking skills. Must have excellent customer service skills and work well with patients, physicians and other community resources. Must have excellent knowledge of home health coverage criteria and service area. Must be highly organized and detail oriented.
