Location: Worcester, MA
Pay: $25/hour (~$52,000 annually)
Schedule: Full-time, Monday–Friday
Overview:
Join a multidisciplinary team providing intensive case management to youth in foster care. This role supports youth and their families through regular communication with providers, schools, and foster homes, while developing and monitoring individualized treatment goals.
Key Responsibilities:
- Build strong relationships with youth and their support networks
- Coordinate with collateral partners (schools, therapists, agencies, etc.)
- Assist in treatment planning and goal development
- Participate in on-call coverage as needed
- Travel locally to foster homes and schools
- Complete MAPP training and become certified to train Spanish-speaking foster parents in medication administration
Requirements:
- Bachelor’s degree in human services or related field
- LSW or LSWA (or license-eligible); must obtain within 6 months of hire
- Valid driver’s license and reliable transportation
- Spanish-speaking ability preferred for specific training responsibilities
