ICE Facility (Needs the ICE form attached filled out)
Must possess the LPC, LCSW, or LMFT from another state with the attached Attestation Form filled out.
Must have an ARC or AHA BLS but if they don’t have it, can fill out the BLS Attestation Form that is attached.
