Position Summary
The Case Manager is responsible for evaluating and directing the care and Supervision of residents in Hope Harbor Center Licensed Residential Drug and Alcohol Treatment Facility. Development and coordination of appropriate activities to meet Evidence Based Practices, entering Patients into HMIS, CAL-OMS, and (DMC) Drug Medi Cal, identify needs and expand on identified interests. He/She will work in collaboration with Clinical Program Manager and Counselors to ensure maximum outcomes in all areas of treatment.
Essential Functions

  • Engage/Coordinate all In-Service Training classes as scheduled.
  • Maintain a caseload of 12-14 clients.
  • Ensure your clients adhere to house rules and log notable incidents.
  • Conduct weekly Case Conference with program Supervisor.
  • Develop and implement Evidence Based Practice curriculum following certification guidelines working closely with Clinical Program Manager.
  • Review charts to maintain efficiency.
  • Ensure all clients maintain current TB tests and physical exams.
  • Report all repairs to Operation Manager and conduct random room checks for caseload.
  • In case of emergencies, follow 9-1-1 procedures, and take the lead to maintain the safety of the clients, staff and facility.
  • Preferable knowledge of (DMC) Drug Medi-Cal, understand six dimensions of (ASAM), ability to create a treatment plan, conduct and document case management, and American Social Addiction Medicine.
  • Responsible for appropriating all treatment services provided for the clients of Hope Harbor Center.
  • Responsible for developing and providing supportive counseling to clients and family.
  • Develop community outreach and oversee referral systems for mental health, medical/dental, vocational and additional therapy needs of the population we serve.
  • Network with community for referrals and resources.
  • Work with Veterans Per Diem Contracts DMC, and IHP/DMH Grants.
  • Perform such other duties as assigned by Director and/or Clinical Program Manager.
Working Conditions
Ability to walk, stand, bend, squat, climb, kneel and twist on an intermittent or sometimes continuous basis. Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead. Ability to operate computer, fax and telephone. Ability to lift up to 50 lbs.
Minimum Qualifications
  • MUST be a certified or registered Alcohol and Drug Counselor (AOD), registration must be up- to date.
  • BA degree preferred in the field of social work/human services.
  • Experienced working with Parolees, Mental Health client, DMC, and homeless population desired.
  • Proficient in Microsoft Word and Excel.
  • Valid California's Driver's License with clean MVR check
Skills, Knowledge & Abilities
  • Excellent oral and written communication skills.
  • Strong organizational skills.
  • Ability to manage multiple priorities simultaneously while paying critical attention to detail.
  • Some Knowledge of Salvation Army policies and procedures preferred.
  • If working in vicinity of children a criminal background check is required with certification for Protect the Mission policies and procedures.
