All social workers shall be licensed or certified by the New Jersey State Board of Social Work Examiners. A social worker shall provide social work services in the facility for at least 30 minutes per week per client, calculated on the basis of the daily census. A Social Worker will be responsible for, but not limited to the following:

1. Check client’s eligibility for admission in Adult Day Care facility.

2. Interview the client and caregiver to obtain a social assessment and evaluation of needs and problems. Complete all admission paperwork, including social service and home assessment.

3. Provide or arrange for individual, family and group counseling in reference to psychological, social, financial, legal, vocational, and educational needs of the client;

4. Coordinate participant’s care plans with other community resources;

5. Assist clients obtain and work on issues related to their Medicaid, Medicare, SSI, SS, housing, food stamp, lifeline services, homecare services and other State and Federal programs.

6. Complete Quarterly Assessment for all clients and participate in Interdisciplinary Care Plan meetings.

7. Document assessments, treatment plans, evaluations and clinical notes;

8. Provide in-service training to staff on participant/ caregiver psychosocial needs;

9. Participate in the facility’s quality improvement program;

10. Coordinate discharge planning for participants.

11. Maintain admission and discharge record.

Experience

A degree in Social Work or a related field is required; CSW/LSW.
Experience in crisis management, behavioral health, and motivational interviewing is highly desirable.
Familiarity with Medicaid processes and regulations is a plus.
Proven ability to engage in effective mentoring and supervising of staff or interns.
Strong communication skills, both verbal and written, are essential for successful client interactions.
