Job Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and advocacy for individuals and families facing various challenges. This role involves working in a hospital or clinical setting, where you will engage with patients, conduct assessments, and develop treatment plans tailored to their needs. The Social Worker will also collaborate with healthcare professionals to ensure comprehensive care.

Duties

  • Conduct thorough assessments of clients' needs and circumstances, including medical, psychological, and social factors.
  • Develop and implement individualized treatment plans based on client assessments.
  • Provide mental health counseling and support to individuals and families, focusing on coping strategies and resource navigation.
  • Document all interactions, assessments, and treatment plans accurately in medical records while adhering to coding standards.
  • Collaborate with multidisciplinary teams to coordinate care and ensure that clients receive appropriate services.
  • Engage in case management activities, including referrals to community resources and follow-up services.
  • Utilize applied behavior analysis techniques when working with children or individuals with developmental disabilities.
  • Maintain up-to-date knowledge of medical terminology and relevant research to inform practice.

Requirements

  • A degree in Social Work (BSW or MSW) from an accredited institution is required.
  • Experience in a hospital or clinical setting is preferred.
  • Familiarity with medical documentation practices and coding is highly desirable.
  • Strong understanding of mental health counseling techniques and case management principles.
  • Experience working with children is a plus; ability to engage effectively with diverse populations is essential.
  • Excellent communication skills, both verbal and written, are necessary for effective client interaction and documentation.
  • Ability to work collaboratively within a multidisciplinary team environment.
  • Knowledge of applied behavior analysis is beneficial for working with specific populations.
