JOIN OUR TEAM AS THE CONTINUING CARE/ALUMNI COORDINATOR!

Oversee and facilitate the Ridgeview Alumni Association. Seeking to build rapport with patients from admission through discharge and beyond so that their experience is positive and life-changing. The goal is to create Active Alumni who will stay connected to RIS by giving back to other patients and families, referring others to RIS program, and being productive members of our community.

The coordinator works under the direction of the Outpatient Director.

Duties and Responsibilities: (Note: The essential job functions of this position are not limited to the duties listed below)

1. Supervise and assist patients in all patient areas.

2. Practice universal precautions and environmental safety guidelines.

3. Recognize and respond to priorities in patient care responsibilities.

4. Conducts one-on-one sessions with patients before discharge, explaining the Alumni Association and Continuing Care programs

5. Facilitates two continuing care support groups per week.

6. Participating in evening or weekend alumni-related projects and functions.

7. Assists to a reasonable degree in cooperative staff planning for effective programming in the building.

8. Acts as liaison between the alumni association and Ridgeview (reviewing minutes from weekly alumni meetings, receiving approval from RIS for alumni association activities)

Your Experience Matters What we’re looking for:

Education: Bachelor's Degree from an accredited college or university in education, social services, or an appropriate discipline.

Experience: One year of related experience, with a knowledge of various teaching methods. Knowledge of addiction patient care techniques with an understanding of addiction.

Licensure: Certification through the State is preferred.

Additional Requirements: Successful completion of CPR certification and CPI to Managing Aggression within 90 days of employment. May be required to work flexible hours.

  • Ability to work with medical office equipment like multi-line phones, computers, printers, scanners, copiers, and fax machines
  • Conflict resolution and mediation
  • Knowledge of Hospice Medicare Conditions of Participation and hospitals
  • Adaptability and flexibility
  • Team collaboration

Compensation:
