Job Summary:

The Care Manager works as part of a collaborative team to support clients living in community corrections. Services include reentry support, including providing information, completing intakes, and connecting clients to essential resources. The Care Manager models the values of Second Chance Center (SCC), which include love, integrity, empathy, social justice, resilience, and service. The goal is to reduce recidivism and help returning citizens reintegrate successfully into the community.

Duties/Responsibilities:


  • Complete intakes and assess participant eligibility for in-house programs.
  • Advocate for criminal justice system clients, focusing on those reentering from Community Corrections.
  • Provide direct care and support to client partners in a community correction setting to aid in their transition towards independence and stability.
  • Connect clients with resources such as transportation assistance, job referrals, clothing, and food.
  • Connect clients with behavioral health and substance abuse support.
  • Assist participants in enrolling in various social programs, including Medicaid, SNAP, and RTD Live.
  • Assist participants in obtaining stable housing.
  • Document client progress in the case management system, including writing notes and tracking goals.
  • Perform regular client check-ins to assess ongoing needs and progress.
  • Develop and maintain care plans that adhere to community corrections standards while upholding SCC’s mission and core values.
  • Utilize motivational interviewing and other basic therapeutic skills to support clients.
  • Attend regular case staffing and collaborate with the operations and care management team on treatment goals.
  • Attend training and professional development sessions to improve client service skills.
  • Participate in team meetings and collaborate with parole offices and community partners.
  • Actively participate in cross-training programs to develop a working knowledge of processes and tasks across related departments.
  • Provide support to other teams or departments as needed, especially during peak workloads, staffing shortages, or projects requiring collaboration.
  • Foster a culture of teamwork by sharing knowledge, skills, and best practices across similar roles to ensure operational efficiency.
  • Demonstrate flexibility and a willingness to adapt to shifting priorities or assignments across departments with similar functions

Required Skills/Abilities:


  • Strong organizational and communication skills.
  • Ability to work in a high-paced and sometimes stressful environment.
  • Familiarity with Microsoft Office (Word, Excel, Outlook).
  • Experience working directly with systemically marginalized individuals.
  • Commitment to SCC’s mission and the principles of diversity, equity, and inclusion.
  • Ability and willingness to cross-train and take on duties in related roles or departments as part of a team-oriented work environment.
  • Strong collaboration and communication skills to effectively work with colleagues across departments.
  • Adaptability to learn and perform tasks outside of primary role as required by business needs.

Preferred Skills / Competencies are as follows:

  • Bilingual (Fluent Spanish) preferred.
  • Previous experience working with the re-entry population.
  • Previous experience in community navigation and service coordination.
  • Understanding of trauma-informed and strengths-based care management practices.
  • Working Environment, Physical Activities, and Equipment Used: Typical office environment. Regularly uses computers for data input and other work.

Required Education and Experience:

  • 0-2 years of relevant experience working in adult re-entry or with marginalized individuals.
  • High School Diploma / GED
  • Lived experience accepted in lieu of education and professional experience.

Physical Requirements:

  • Occasionally required to stand, walk, and lift up to 20 pounds.

Compensation & Merit Increases:

  • The pay range for this position is $24.04 - $26.44 per hour.
  • Competitive pay commensurate with education and experience.
  • Vacation Leave & Sick Leave, Medical, Dental & Vision Insurance, 401k Plan.
  • Merit increases are based on annual performance reviews.
  • Cross-training and departmental support are integral parts of this role and may be assessed as part of ongoing performance evaluations.
  • Employees brought in at the top of their pay range will need to transition to a role with greater responsibilities to qualify for further raises.

Position Type/Expected Hours of Work

  • This full-time position requires a minimum of 40 hours per week.
  • Some evening and Saturday work may be required.
  • Work hours are 8:00AM – 4:30PM, Monday through Thursday, and 8:30AM – 12:30PM Friday & Saturday, but may be adjusted according to business needs.
  • This role is in-office.
