MAIN FUNCTION Serve as the first point of contact for potential clients, families, and referral sources (EAPs, detox facilities, etc.). Facilitate appropriate admissions, assess medical eligibility, and refer as needed. Utilize assigned information systems effectively.
ESSENTIAL DUTIES AND RESPONSIBILITIES
•
Coordinate client admissions with referral sources (e.g., medical facilities, outpatient programs, EAPs, unions)
•
Answer inquiries and explain Seafield's admission procedures and programs
•
Schedule assessments at outpatient offices
•
Submit and follow up on insurance verifications; assist clients with coverage issues
•
Complete and prepare admission documentation, including EMR entries, PAS forms, and consent forms
•
Liaise with Utilization Review, Medical, and Transportation departments
•
Maintain accurate bed board and coordinate bed placements
•
Review hospital records for placement suitability; refer to alternate care levels when appropriate
•
Build and maintain referral relationships; follow source-specific protocols
•
Provide facility tours, collect payments, scan/upload client documents
•
Perform additional duties as needed
COMPETENCY REQUIREMENTS
•
Excellent communication and decision-making skills
•
Proficient in EMR, Microsoft Outlook, and Medicaid systems
•
Strong understanding of insurance processes and placement guidelines
•
Ability to assess psychiatric status and work independently
•
Effective time management and attention to detail
•
Knowledge of inpatient/outpatient services and 12 Step programs
EDUCATION AND QUALIFICATIONS
•
NYS Licensed Practical Nurse (LPN) preferred
•
Minimum 1 year of experience in substance abuse treatment
•
Strong phone and computer skills; ability to work independently
PHYSICAL DEMANDS
•
Must be able to talk, hear, and use a computer and office equipment
•
Primarily sedentary with occasional walking/standing
•
Requires close and distance vision, and ability to adjust focus
