Divine Living Homecare is hiring a passionate and experienced Community Liaison to drive our outreach and referral growth strategy. You’ll represent our agency in the community—building trusted relationships, presenting our services, and expanding our referral network with senior care professionals.

Key Responsibilities

• Build and manage referral relationships with hospitals, rehab centers, assisted living facilities, and social workers
• Promote our home care services to discharge planners, case managers, and community professionals
• Identify new referral opportunities and support geographic expansion
• Collaborate with agency leadership to meet monthly growth targets
• Track referral activity and provide insights to improve outreach performance

What We’re Looking For

• Proven ability to grow a home care agency’s census or referral base
• Excellent communication and relationship-building skills
• Self-starter with strong organizational and time management abilities
• Valid driver’s license and reliable transportation for local travel
