LICENSED SOCIAL WORKER, Home Visiting, Mother/Baby Program. Positions are available in the cities of Wayne, Westland and Garden City (must be available to cover all three cities).

My Pregnancy Coach (www.mypregnancycoach.org) is hiring licensed SW's to manage a caseload and conduct home visiting support services for the Maternal Infant Health Program (MIHP), as provided by the state of Michigan (www.michigan.gov/mihp). Experience is NOT REQUIRED. If you DO have experience in MIHP, give us a call and see what makes us different.

This position is a rewarding opportunity to work with a client base of pregnant women and infants. A risk assessment and client input will guide you in providing the support, counseling, and resources needed to promote a positive pregnancy outcome and a healthy baby. You will conduct half-hour, pre-scheduled visits in the homes of your clients. Documentation is paperless, using an online program - electronic medical record (EMR) format.

MUST BE COMPASSIONATE, ORGANIZED AND DEPENDABLE.

This is a contract position with a required minimum commitment of 15 visits per week. Potential for 40+ visits per week. Visits are 30 minutes in length.

Pay $35 per visit

REQUIREMENTS:

-- current Michigan licensure as a licensed SOCIAL WORKER (limited license is acceptable) by the Michigan Department of Licensing and Regulatory Affairs

-- valid driver's license

-- Michigan Department of Health and Human Services MIHP required training. (We will assist with direction on completing this online training. Approximately 25 hours).
