This position is a grant-funded position expected to last for up to eight (8) years. The Choice Neighborhoods Implementation Grants support those communities that have undergone a comprehensive local planning process and are ready to implement their “Transformation Plan” to redevelop the neighborhood.
Position Specific Summary
The Resident Services Care Coordinator position at the City of Tucson's Department of Housing and Community Development is responsible for providing resident services and case management. This position will ensure that public housing residents have access to comprehensive educational, employment, recreational, wellness, health, and social services resources.
Work is performed under the supervision of the Community Services Project Supervisor. This position does not supervise.

Duties and Responsibilities
  • Maintains an active caseload and delivers case management and service brokerage to residents by identifying, coordinating, and monitoring access to community resources and social services tailored to individual needs. Coordinates and promotes resident participation in on-site programs, services, and activities that support well-being and self-sufficiency. Participates in collaboratives and professional development opportunities relevant to assigned service areas.
  • Facilitates and monitors referrals to community-based services, ensuring residents’ needs are met and follow-up is completed as necessary. Conducts annual or as-needed resident assessments to determine service needs and eligibility for various programs.
  • Plans and conducts on-site educational, wellness, and recreational programs to enhance residents’ health and quality of life. Shares and researches best practices and community resources related to diverse resident populations, including families with children, older adults, and individuals with disabilities.
  • Maintains resident service files, records and documentation in accordance with City policies and federal requirements including privacy and confidentiality standards (e.g., HIPAA, if relevant). Collects and compiles data for grant applications, compliance reports, and local, state, and federal reporting obligations.
  • Assists in piloting new practices, services, and programming. Partners with redevelopment and relocation teams to educate residents on their rights, responsibilities, and opportunities during relocation, supporting successful transitions and maximizing re-occupancy of redeveloped units.
  • Performs all other duties and tasks as assigned.
Working Conditions
Mostly office environment.

MINIMUM QUALIFICATIONS
Education
Bachelor's degree
Experience
Two (2) years of directly related experience
Equivalency
Any combination of relevant education and experience may be substituted on a year-for-year basis.
License/Certifications
Valid and unrestricted Arizona Class D Driver License - Arizona Department of Transportation - Motor Vehicle Division
Preferred Qualifications
Degree in Social Work, Human Services, Public Health, Psychology, Sociology, or a closely related field.

Experience in case management, care coordination, or social service delivery, preferably in affordable housing or community-based settings.

Experience working with diverse populations, including individuals with disabilities, older adults, and families with complex needs using a trauma-informed approach.

Bilingual skills are highly desirable (English/Spanish)
