A growing healthcare organization in Crown Heights is seeking a part-time Service Coordinator to manage a caseload of up to 25 patients. This position involves coordinating support services, maintaining accurate records, and ensuring that all documentation and compliance requirements are met. The ideal candidate will have strong organizational skills, be patient-focused, and capable of handling responsibilities independently.
Responsibilities Include:
Manage case files and coordinate services for up to 25 patients
Arrange transportation and order medical supplies as needed
Ensure monthly and annual paperwork is completed accurately and on time
Conduct quarterly in-person visits with patients to assess and support their needs
Maintain consistent documentation and meet all compliance standards
Work collaboratively with other care team members
Ideal Qualifications:
Associate’s or Bachelor’s degree required
Previous experience in case management or healthcare support is a plus
Strong communication and interpersonal skills
Highly organized and detail-oriented
Able to manage time effectively and handle sensitive situations professionally
