Una Health is seeking an experienced Registered Nurse to join our team as a Care Management RN Supervisor, supporting Medicare patients through our comprehensive care management services, such as Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Advanced Primary Care Management (APCM).

In this pivotal role, you will not only provide direct patient care but also mentor and lead a team of care managers, build strong clinic partnerships, and help develop patient engagement workflows that drive improved outcomes across multiple care management programs.

Key Responsibilities

● Provide direct care management to an assigned panel of patients for approximately 3 to 6 months as needed

● Hire, onboard, and supervise a team of care managers to ensure quality care delivery

● Review patient care plans and provide feedback, coaching, and clinical expertise to care managers

● Collaborate with Una Health leadership to develop and oversee workflows for patient engagement, enrollment, and care coordination

● Serve as the primary communication liaison with partner clinics to resolve patient issues and ensure alignment

● Meet with providers and clinic staff during program onboarding and ongoing training

● Conduct on-site visits as needed to drive strong clinic relationships and optimize patient engagement

Required Qualifications

● Active Registered Nurse (RN) license

● Minimum 1 year of experience in care management, chronic disease management, or case management

● Proven experience mentoring or supervising clinical or care coordination teams

● Excellent clinical assessment, communication, and coaching skills

● Proficiency with electronic health records (EHR) systems and documentation workflows

Preferred Qualifications

● Experience in Medicare value-based care programs such as CCM, RPM, APCMp, or population health services

● Familiarity with care coordination models and patient engagement strategies

Pay: $51,462.00 - $55,986.00 per year
